Frequently Asked Questions

At Cottonwood Family Medicine, we want our patients to feel informed and at ease.

Services & Appointments

How quickly can I be seen by a provider?

New patients are typically seen within two to four weeks. For established patients, we offer same-day and acute appointments whenever possible.

Will providers complete Family Medical Leave (FMLA) forms?

Yes, for established patients only and at the provider's discretion. Completed forms require a $45.00 filing fee. There is a two-week turnaround time to receive the forms back.

Does CWFM manage chronic pain?

Our team offers pain management based on professional judgment. If we are not the appropriate clinic to manage your medications, we will gladly arrange a referral for you to see a dedicated pain specialist.

Do your providers file disability claims?

We may file disability claims for established patients when the provider recommends that the patient apply. This is determined on a case-by-case basis.

Does CWFM manage psychiatric medications?

CWFM manages psychiatric medications within the scope of a CNP’s practice. Our providers diagnose and treat many mental health conditions, offer counseling referrals, and can manage some complex medication plans that have been stable for at least a year. When your needs are best served by a mental health specialist, we’ll refer you to an appropriate provider.

If you ever have thoughts of harming yourself or others, call 911 or go to the nearest emergency room. CWFM is not a 24/7 crisis service.
